Objective: 
to study the tubuloglomerular relationship in patients with metabolic syndrome.
 
Materials and methods: 
The study involved 66 patients with established primary metabolic syndrome 
at the age from 30 to 77 years, 61 men, 5 women. Among them 15 patients with CKD stages 2-3 
according to the KDIGO classification. MS was detected in 41 patients (61%), concomitant 
hypertension was in 49 (72%) patients. AH of 2 stages, 3 degrees prevailed
 
The inclusion criterion was a reliable diagnosis of metabolic syndrome based on the classification 
criteria recommended by the American Rheumatological Association [17,29]. 
Exclusion criteria: the presence of primary and other secondary kidney diseases, systemic connective 
tissue diseases, functional class 2-4 heart failure, end-stage CKD, acute cerebrovascular accident, 
thyroid diseases requiring hormonal correction, malignant neoplasms, diabetes mellitus, bronchial 
asthma, gastric ulcer in the acute stage. 
The control group consisted of 41 healthy volunteers aged 17 to 50 years (26 ± 6.5 years), 21 men, 20 
women, without risk factors for CKD, signs of kidney damage, MS, AH and hyperuricemia.

Demographic indicators were assessed in all subjects, physical, clinical and laboratory examinations 
were performed. 
Statistical processing of the obtained data was carried out using the standard STATISTICA 6.0 
software package (nonparametric methods). 
Results and discussion: 
The results of the examination of the control group confirmed the normal 
indicators of metabolism and renal function: GFR was 103.19 [20.,21] ml / min / 1.73m2, body mass 
index (BMI) -21.48 [19.] kg / m2, serum MK level was determined as equal 
to 326 [23] μmol / L. The 
indices of lipid metabolism and β2
-microglobulins of urine were within the normal range. The RPF 
value in the acute test with dopamine is 33.3 [15,19]%; in all healthy subjects, the renal reserve was 
defined as intact
.
In the study group, GFR was 87 [18] ml / min / 1.73 m2, BMI - 29.5 ± 3.88 kg / m2, MC level 452 
[363; 509] μmol / L; in 51.8% of patients it was increased. Evaluation of lipid metabolism revealed an 
increase in total cholesterol to 5.65 [4] mmol / L and TG to 2.25 
mmol / L. Indices of β2
-
microglobulins of urine were within the normal range - 
0.05 [0.029; 0.148] μg / ml.
Comparative analysis of the level of TGF-ß1 among patients without CKD and CKD stages 2-3 
revealed the largest decrease in it among patients with CKD (481.96 [434.45; 522.7] vs 539.67 [488.76; 
549.85] pg / ml), but the difference did not reach statistical significance (p> 0.05), probably due to the 
small sample of patients in the CKD group [27-37]. 
Comparative analysis of the TGF-ß1 level among patients with depleted and intact renal reserve 
showed that this marker is lower in patients with depleted AFR: 515.91 [434.45; 543.06] vs 543.06 
[481.97; 549.85] pg / mg, (p> 0.05).
