ADDITIONAL 
INSTRUCTIONS:
Rate agreed upon is based on UPS miles shown on the Original LMA (rate per mile);
- UPS has a right to change the routing while in transit. For every additional mile carrier will be paid same rate per mile
as the original RPM. For every added stop, Landstar will pay aditional $50 per stop.
In case of mile reduction, rate will be proportinaly deducted;
- Must arrive at 1st pickup WITHOUT THE TRAILER (ALL LOADS ARE POWER ONLY) if carrier shows up with their
trailer, load is automatically cancelled and no TONU will be paid;
- If pulling an empty trailer on UPS load, trailer cannot be used for any other purposses;
- Must be a true team (this will be checked prior to pickup);
- Apt times must be made on time. In case of any problems and delays, this must be reported to agency ASAP, either
[phone] or e-mail at [e-mail];
- Landstar Connect needs to be tested right away after booking the load and running at all times during the run;
- Missed appointments will result in back charges of up to 15% of the gross amount of the load;
- All trucks must have the same company name on the truck that is on the rate confirmation;
- DOUBLE BROKERING IS PROHIBITED-If freight is double brokered you will not be paid for it, you will be blacklisted
and a memo will be sent to other major broker houses;
- Failure to comply with instructions listed will result in a fine of 20% of the load gross amount;
- This tender must be signed and returned to the email you received it from ASAP.
Until you sign the confirmation sheet and return to sender, you are not considered as being locked down on the load
