DIRECTORY OF NETWORK PROVIDERS

PALM BEACH
All service providers included in this directory have been reviewed by ChildNet Service Coordination and Contracts staff. Any programs or providers that ChildNet pays have been credentialed by Contracts & Licensing staff based on their meeting basic requirements mandated by Florida Statute or standards described by the Council on Accreditation. Additionally, all anger management, parent skills education, domestic violence treatment, mental health counseling, and substance abuse treatment providers included here have been approved by Service Coordination staff for use in dependency case plans based on a consideration of key program elements including curricula, staff credentials and, where appropriate, licensure.

Instruction to Dependency Case Manager (DCM): For any behavioral health or tutoring service that ChildNet is being considered as the payer, a General Funding Authorization and Purchase Order and a referral form must be submitted by the DCM to the Administrative Assistant of Service Coordination (AA) either via email or hand delivery to office 215C.
Do not send referral forms, when ChildNet is the intended payer, directly to the provider in the absence of a General Funding Authorization and Purchase Order or with an unapproved General Funding Authorization and Purchase Order. Upon approval of the expense by the Director of Service Coordination, both the General Funding Authorization and Purchase Order and the referral form will be sent to the provider by Service Coordination, with a cc: to the DCM, DCM Supervisor, and Program Operations Director.
General Funding Authorization and Purchase Orders submitted without a referral form cannot be processed by Service Coordination. Additionally, some providers require consent for treatment forms or other documents to be submitted with the referral form. This directory will provide that information.
Behavioral Health Service Specialists are available to assist DCM is locating services the meet the client’s needs. Prior to making a referral and/or completing a General Funding Authorization and Purchase Order (GFA) and referral form for services ChildNet is being asked to pay, Behavioral Health Service Specialists are available to answer any questions or assist in finding specialty services not outlined in this directory. Additionally, two of the larger provider agencies have been assigned Behavioral Health Service Specialists to act as the primary liaison between provider and DCM.
