Where there is a clear need for the introduction, or enhancement of telecare, early 
referral should be made, well in advance of discharge

Referrers should have a knowledge of telecare and an awareness of referral 
processes, and liaise with TEC/telecare service as required 

Referrals should contain the right level of detail to allow timely and appropriate 
installations; liaising with families and carers 

The patient, and their family and carers, where appropriate, should be made fully 
aware of what telecare is, and that there is a charge
