G. REVENUE GENERATING CONTRACTS FOR SERVICE

It is recommended the Board approve the following Revenue Generating Contracts for Service and that the Executive Director and Board Secretary be authorized to execute same on behalf of, and as agent for, the Board of Directors of CIU-20:

CIU20 will provide Behavioral Specialist Consultant services to students in the ESASD

Shall not exceed $1,341.50

April 10, 2015 – the end of the 2014-2015 school year







Justification for lateness: Request for services occurred after the March Board meeting and prior to April’s Board meeting

Easton Area School District

CIU20 will provide an Associate Teacher for direct, one-on-one services at the following at George Wolf Elementary School in the Life Skills Support program

Not to exceed $9,886.08

April 6, 2015 – the end of the 2014-2015 school year







Justification for lateness: Easton School District was providing a 1:1 Associate Teacher during a transition period of the student over to an IU20 classroom. We were notified on 4/1/15 that as of 4/6/2015 the student would be transitioned over to CIU20 Instruction.

Easton Area School District

CIU20 will provide Applied Behavior Analytic (ABA) services to students in the EASD

Shall not exceed $965.88

May 1, 2015 – May 31, 2015

Nazareth Area School District

CIU20 will provide Schoology, a digital learning environment that promotes learning, communication and collaboration inside and outside the classroom

Shall not exceed $23,320.00

July 1, 2015 – June 30, 2016

Northampton Area School District

CIU20 will provide Applied Behavior Analytic (ABA) services to students

Shall not exceed $1,341.50

Justification for lateness: PHP Supervisor not aware of the need to do another contract upon transfer of student’s 1:1 to different program

Will be based on the total number hours requested at $46.09/hour plus CIU20’s mileage reimbursement for travel policy.

Justification for lateness: When the contract was forwarded to the Business Office it was inadvertently put in with the wrong paperwork and didn’t get forwarded for the agenda.

CIU20 will provide an associate teacher for direct, one-on-one services at the Washington Elementary School in the Autistic Support program

Shall not exceed $9,474.16

April 13, 2015 through the end of the 2014-2015 school year







Justification for lateness: The request for services was received after the previous Board Meeting, but services needed to start before the April Board Meeting

Northampton Area School District

CIU20 will provide Schoology, a digital learning environment that promotes learning, communication and collaboration inside and outside of the classroom

Shall not exceed $25,740.00

July 1, 2015 – June 30, 2016

Pocono Mountain School District

CIU20 will provide two associate teachers for direct, one-on-one services in the Autistic Support program at the following locations: Pocono Mountain East Junior High School and Swiftwater Elementary Center.

Shall not exceed $25,127.12

March 30, 2015 until the end of the 2014 – 2015 school year
