Accessible Information Standard

The Accessible Information Standard is a new law that asks NHS and Adult Social Care organisations, including South Park

Surgery, to make all reasonable adjustments for Patients with a disability, impairment or sensory loss by providing information in a format that they can easily access and understand.
If you have any communication support needs, please inform a member of our Team, so that we can take all reasonable steps to provide information in the most helpful and appropriate

format for you, such as:
