When driver calls

DISPATCH INSTRUCTION Dear CARRIER!
United freeway transportation LLC asks you to confirm details of the transportation with the customer BEFORE YOU GO TO THE PICK-UP ADDRESS.
Please confirm with the customer:

• 
  Agreed amount of COD
  
• 
  Exact pick up address
  
• 
  Exact delivery address
  
• 
  Scheduled time for pickup and delivery
  

The information added by United freeway transportation LLC on the system is correct and confirmed by customer. If the pickup or delivery of the shipment is delayed by the carrier, the party will have to pay $25 late pickup or late delivery fee for each day of delay.
By signing the dispatchsheet the carrier agrees not to cancel the load, otherwise $50 cancellation fee has to be paid to United freeway transportation LLC by the carrier.
CARRIER shall defend, indemnify and hold United freeway transportation LLC harmless from any claims, actions or damages, arising out of its performance under this Agreement, including shipment (vehicle) loss and damage, theft, delay, and personal injury or death. The obligation to defend shall include all costs of defense as they accrue. It is CARRIER’S responsibility to collect the COD payment at the delivery.
