To be considered a complete application, this packet must be filled out in its entirety and submitted with:

2 Letters of Recommendation

Principal, Counselor or Advisor

Classroom or Homeroom Teacher

Pastor or Out-of-School Activity Sponsor

Copy of official High School Transcript (provided by Guidance Department)

Applications must be received by the West Allis~West Milwaukee PTA Scholarship committee on or before February 17th, 2012. Applications will be picked up at the end of the school day on February 17th, no late applications will be considered.

All questions on the application must be answered completely. If a question does not apply to you, please write “N/A” or “None” in the space provided.

Applications must be typed or computer generated.

Applications must be signed and dated.

Applications not meeting all of the above requirements can be disqualified from consideration.

Who May Apply?

Any student graduating from West Allis Central, Nathan Hale or James E. Dottke High School may apply. Up to six scholarships will be awarded. All applications will be evaluated on their merit.

Benefits

$600.00 scholarships will be awarded (one half at the beginning of each semester) to each recipient. These funds are provided to the students to be used toward expenses at any college, university or technical college. Student recipients must complete at least one full year of his/her education, or the money must be refunded to the West Allis~West Milwaukee PTA Council.

Basis of Selection

Scholarship recipients will be selected based on the criteria of: leadership, character and written application as well as any other information provided by the student.

How to Apply

1. Obtain application form in the school’s Guidance Office, complete and return form to the Guidance office, on or before February 17th, 2012. No late applications will be considered.

2. Submit your application with two (2) letters of recommendation. Letters of recommendation may be written by:

a. School principal, counselor or advisor

b. Classroom or homeroom teacher

c. Your pastor or out-of-school activity sponsor

3. Have your school counselor or the guidance office attach a copy of your high school transcript credits to your application.

4. All information supplied will be treated in the most confidential manner possible.

5. All entries must be typed or computer generated. If you are using a computer without a scanner, you may attach information to the application form.
Please be sure to answer all questions completely.
