The character of respiratory murmurs: vesicular (normal, decreased, increased, hourse), bronchial, amphoric, mixed, the absence of respiratory murmurs with exact description of the bordersfor every type of respiration. The presence of side respiratory murmurs (rales: dry of low tones high tone. Moist rales: clear, unclear, small-, middle-, large-, cysted. Crepitation) To define exactly the areas of every type of side respiratory murmers. Bronchophony; the conduction of voice murmurs equal from both sides, decreased or increased from the one side. To point out the location of the revealed changes.


Cardiovascular System
Complaints (presense, absence).

1. 
   Breathlessness: constant constant (at rest) or during physical
   

axertion (walking, going upstairs, on any movement, speaking, depending on low horizontal position in bed), intensity duration of breathlessness.

2. 
   Pains in cardiac region (constant or paraxismal), location (behind
   

the breastbone, in the region of apex, beat etc.) character: aching, stabbing, compessing dull associating with the feeling of depression and fear, intensity and irradiation; duration frequency of pain attacks, reasons and conditions of pain appearance (on physical exertion, excitement, during sleep, etc) patient`s behavior during the attacks of pain.

   Fibrillation, failure of the heart: constant or periodical, attacks of
   

paroxysmal trachycardia.

4. 
   Heart beating: character (stable, paroxysmal, intensity, duration);
   

conditions of appearance – on physical exertion, during diarhoea, after meals, on changing body position, on excitement,etc.

5. 
   Edema, their persistence, location (on legs, in other places), time
   

of appearance (in the morning, evening, etc.), association with physical exertion, drinking fluids.

6. 
   Filling the pulse (in what parts of the body).
   
7. 
   Signs of the spasm of peripheric vessels, intermittent lameness,
   

feeling of “dead” finger, etc.

8. 
   Symptoms of cerebral circulation disturbances, weakness of the
   

limbs, convulsions, disturbances in tenderness; disturbances in the functions of sense organs, speech, etc.
