It is medical biography of the patient according to the main periods of life.
Where, when was born and what child in the family.
Social conditions of life in childhood, youth, adult. Development
of an organism in childhood, general condition of physical and mental abilities in the beginning of the school period. If the patient finished school in time. If not, why?
When did the patient begin to work: character of job from the
beginning of labour activity and following changes. Gradual changes of labour activity and address, reasons.
Conditions of labour and life during the last period of life:
Conditions of labour – profession, hygienic conditions, mode of work, additional loadings, rest (weekends, periodic holiday);
Living conditions;
Character of a feed, qualitative and quantitative characteristics and
regularity.
Sexual and home life, marital status (single, married, divorced).
When did the patient’s menstruation begin, their character, regularity, volume of the blood (little, moderate, plentiful), morbidity (painful, poorly painful, acute pain, painless), delays and discontinuance. Household anamnesis and heredity, diseases and bad habits of the
parents, wife (husband), family members, children and men living with the patient. If somebody died – causes: (cardiovascular diseases, tuberculosis, syphilis, alcoholism, psychic diseases, cancer, disease of the exchange, hemophilia and etc.)

11. Insurance anamnesis: frequency of using the disability sheet, if
the patient is invalid. At the moment from what time the patient has the sheet of disability.
12. Allergist anamnesis. Allergist diseases in the family, before and
now (at grandmothers, father, mother, brothers and sisters). Reactions on blood transfusion (when), entering the whey of the vaccines (what and when), different medicines (what and when). Time of the disease (summer, autumn, winter, spring), influence of the weather (overcooling, overheating and etc). The connection with following diseases (tonsillitis, bronchitis, sinusitis and etc.). where and when the fits “pp” occur frequently and the change of the condition for the worse (at home, at work, on the field, at night, daytime and etc.) the influence on the course of “pp” :
Different food products, alcohol, cosmetics, dust, smells;

13. Epidemiological anamnesis. Whether the contact with
inflectional people (in a family, at school, at office, on the way, among relatives and etc.). took place. Whether he contacted, at least for a short time, with the people especially with fever.
Whether he contacted with diseased animals or the corpses and carcasses of animals. Whether he was stung by insects” lice, bedbugs, mosquitoes and others. The presence of parasites and rodents in the house. Possibility of infection in the connection with the occupation, working activities, using of water, feeding, using of closes and shoes and other. Whether he has been somewhere away for the last 1,5-2 months, when exactly, where and how long.
The following of personal hygiene (washing, change of underwear and other). What prophylactic vaccination he has taken, when and how many times. Epidemiological anamnesis – to indicate the possible connection and the data of epidemiological anamnesis.
