Muscular system:

Pain, its character, localization. Mobility: active, passive, contractions, deformation. Swelling, thickening, deformation. Pain in palpation during active and passive movements: crunch, fluctuation, local high temperature, skin color above the joints.


RESPIRATORY SYSTEM
Complaints: (yes, no)

   Cough – dry or with sputum: time of coughing (in the morning,
   

during the day); permanent or spasmodic, reasons causing and strengthening the cough.

2. 
   Sputum: the way of discharging (easily, hardly, in what position
   

discharges more), amount and fluctuation during the day; character (mucous, serous, purulent, bloody, mixed) smell; color.

3. 
   Blood sputum: how often repeats the amount. High temperature.
   

The time of occurrence, intensity, bloody streaks or dots in the sputum, sputum mixed with blood, in the amount of a few spits, the full volume of a cup or a glass, liquid or thick; color: scarlet, dark, black.

4. 
   Pain in the thorax: localization, character, intensity, permanent or
   

spasmodic the reasons for strengthening, irradiation of pains.

5. 
   Edema (degree, character, time and reasons for occurrence; types:
   

inspirational, expirational, mixed)
