The City of South Sioux City, coaches, parents and workers, and the SSC Post 307 Baseball / Rick Voss in making this recreational activity available for your participation, assumes no responsibility for injury. The responsibility for injury is assumed entirely by the participant. Participants in recreational activities are NOT covered by any special insurance coverage; therefore, participants should have adequate insurance coverage. I have notified all the parents or legal guardians of the players listed below and we accept the responsibility as stated above. In addition, I certify that all players listed are properly represented and are in the appropriate division.
