Maintenance request form:
Form used to initiate and document a request for maintenance.
Trained maintenance and engineering personnel know that, of the three potential
problems just noted, the problem with electronic door locking is the priority. The room
cannot be rented until that repair is completed. Perhaps the television set can be repaired
on-site, or it can be exchanged for another set in the hotel’s inventory. The dripping
faucet probably can be repaired quickly. However, the number of and length of time
required to fulfill maintenance requests in other rooms affects the time needed to com-
plete work on a specific room. Typically, the department that initiates the out-of-order
room status is the same department that will reinstate the room. For example, if the
maintenance department places the room on out-of-order status, it will be responsible
for placing the room back in order after necessary work has been completed.
