Summary of proposed apprenticeship and research plan
(three pages maximum) – 
This document should describe the proposed apprenticeship research focus, activities 
and methodology, as agreed upon by the aspiring apprentice and host organization. The 
link between proposed activities and one or more of the RAP priority areas listed above 
must be clearly explained.

Overview of relevant organizational past performance 
(two pages maximum) – 
This should outline what current or recent projects the organization has undertaken 
with a similar focus to the proposed apprenticeship

Resume of apprentice
(two pages maximum)

Resume of the mentor 
(two pages maximum)

Letter of support from the host organization – 
A letter on official company 
letterhead and signed by an executive of the organization confirming desire to 
participate in RAP and should confirm nomination of the selected mentor from among 
organizational staff. 
7.
Letter of support from the apprentice
– A signed letter from the apprentice 
expressing their interest to work at the proposed host organization.
If additional information or clarification on applications is needed, the applicant(s) will be 
notified in writing and asked to submit additional information and/or documentation.
All application documents must adhere to the following guidelines. Applications that do not 
follow these requirements may be considered technically ineligible:
