Participation Release:

My child, ________________________________has permission to fully participate in the Encore afterschool program at East Lynne school.

Student/Parent Contract

Participants of the Encore afterschool program must read and accept the following statements in order to make their participation rewarding and beneficial.

1. 
   I will come prepared to devote one hour each day to academics.
   

2. 
   I will meet the minimum attendance requirement of the program, understanding that daily attendance is preferred. My lack of attendance will result in my termination from this program.
   

3. 
   I will follow all school rules as they also apply to the Encore afterschool program.
   

4. 
   I will report on time each day at the designated area assigned by the Encore afterschool staff.
   

5. 
   I understand that Encore staff will access school records to provide individualized services.
   

6. 
   Fighting, misconduct or unwillingness to participate or follow staff instructions can result in my immediate suspension from the program.
   

7. 
   As the parent/guardian, I agree to pick my child up before 5:00 p.m. if they are not a bus rider.
   

8. 
   I will keep the staff informed of any changes or important information that affects my child's well being.

The Encore Program is an academic based after-school program, allowing students to improve their academic performance staffed by certified teachers.

Program Operates

Monday – Thursday beginning immediately after school dismisses until 5:00 p.m.

  To provide educational experiences and enrichment programs for students who need reinforcement, exposure to a wider view of the world, or non-competitive opportunities to develop intellectual skills and foster positive peer interaction.
  

Cost of Program

The Encore Program is generously supported by East Lynne School District and federal programs. Donations are always welcome. To keep Encore free for our students, please consider making a voluntary, tax deductible contribution to the program. Thank you for any assistance you choose to provide.

Program Closing

In the case of severe snow/ice/heat or other emergency, District Administrators may decide to close Encore early. If this should happen, parents/guardians will be notified via school website and television stations. We will make every attempt to keep our program open.
End of the Day

Students riding the bus home will dismiss from the gym doors. Students picked up by parents will dismiss from the Middle School hallway.
