I hereby give my permission for __________________________ to participate in the ______________________________program. I relieve the Southwest Greensburg Recreation Board, managers, coaches, officials and instructors of all responsibility in the undertaking of this program.
I will accept full responsibility and care of uniforms and equipment issued to my child, and will reimburse the Southwest Greensburg Recreation Board for any damages or loss of the uniforms or equipment. There will be $50.00 charge assessed for any uniform not returned.

$25.00 (+ $25.00 Concession stand fee)
Make out two (2) checks for $25.00 each, one will be cashed the second will be returned upon your concession stand fulfillment. Please make check payable to: SWG Rec.
Any parent having more than two (2) children in a particular program at one time need pay only for the first two (2) children. All others are registered free.
Programs will be filed in the following priority:

Due to the rising cost if insurance, the Southwest Greensburg Recreation Board will no longer provide coverage for participants in their programs. Therefore, it is necessary that each parent understands and acknowledges that, and checks the box below.
I have provided other insurance coverage for my child which I feel is adequate and consent to use
that policy to cover my child/children.
CONCESSION STAND FEE
This year, we will be implementing a concession stand fee. Each child that participates in the program will be required to pay an additional $25.00 “Concession stand fee”. This fee WILL be refunded once parents/guardians work their allotted times in the concession stands. By doing this, we can now “staff” the concession stands and parents/guardians have the choice to work to receive their $25.00 fees back. This is NOT optional, and is mandatory to participate.
