Dear Property Owner:
The Infrastructure Services Division is preparing plans for the reconstruction of South Howell Avenue between Layton Avenue and Howard Avenue. This project will involve a reconstruction of the existing roadway. As part of this project, we will be replacing the northerly driveway approach at 4358 South Howell Avenue, tax key number 594-0470-100, with a new concrete driveway approach. It is necessary that you give our office complete information relative to the width and location of the driveway approach. The approach will be built at the same time that the street is constructed.

This application must be RETURNED WITHIN TEN (10) DAYS to complete our records.
INSTRUCTIONS
1. If you want your new driveway approach to match the width and location of your existing driveway write "SAME" in box "A".
2. If you desire a change in your driveway approach width, write the new width of your driveway approach in box "A". The new approach will be centered on your existing driveway.
3. If you want to change the location of the driveway approach please call our office at [phone] between 8:00 a.m. and 4:45 p.m.
NOTE: The driveway approach will be approximately six feet wider at the curb than at the public walk except for unusual field conditions.
