The scientific novelty of the research work is as follows:
the main pathogenetic and clinic risk factors for conversion appendectomy are specified depending on various manifestations and features of complicated acute appendicitis clinical course in children;
significant intraoperative factors were determined during laparoscopic interventions in children with acute complicated appendicitis, the establishment of which increases the risk of conversion to open appendectomy;
high informativeness of computed tomography is shown in identifying not only signs confirming the complicated course of acute appendicitis in children, but also predictors of conversion appendectomy;
the importance of determining perioperative conversion predictors in reducing “negative” laparoscopy in children with acute complicated appendicitis has been proved.
Introduction of the research results. According to the results of a scientific research to optimize the surgical treatment tactics of acute complicated appendicitis in children:
methodical recommendations «Laparoscopic surgery of acute complicated appendicitis in children» (certificate of the Ministry of Health number 8n-z/56 dated March 30, 2021). The developed recommendations made it possible to improve the treatment results of pediatric patients with acute complicated appendicitis, especially in the group of patients with gangrenous/perforated form of disease;
the obtained scientific results have been introduced into the practice of healthcare, in particular, in the pediatric surgery department of the Navoi and Khorezm regional multidisciplinary medical centers (certificate of the Ministry of Health number 8n-z/56 dated March 30, 2021). Improving the tactical aspects of acute complicated appendicitis surgery in children has made it possible to increase the laparoscopy activity from 59.2% to 94.2%, reduce the rate of conversion appendectomy from 18.2% to 10.6%, rate of specific complications from 40% to 7.9%, increase the incidence of good results from 57.7% to 78.3% and reduce the incidence of poor results from 9.2% to 2.5%.
