You were selected to respond to this survey because it is expected that you have the relevant information about your campus. If not, please give the enclosed questionnaire to the appropriate person.
The questionnaire has been designed so that you can complete it in a few minutes. For most questions, you need only check the appropriate box. A postpaid return envelope has been included for your convenience.
The survey results will be used in a research report. Questionnaires have been coded by school for followup purposes. Neither individuals nor institutions will be identified in my report. Your answers will be combined with those of others and used only for statistical analysis.
