Investigation of oral cavity:
Smell: common, putrid, of faeces, ammonia, “apples” and so on.Mucous membrane of inner surface of lips, cheeks, soft and hard palates, its color: normal, pink, pale, reddish. Pigmentation: Filatov’s spots. Leukoplakia. Gums: pale, pink, loosened, haemophilia, grey edging. Teeth: decayed, loosened, absent (where). Getchinsonov’s teeth. Tongue: moist, clean, dry, coated. Swelling of the tongue, cracks, ulcers. Pharynx: of normal color, redness, swelling of mucous membrane, dryness, thin coating.
Tonsils: of normal size, redness, swelling, plagues (with description of their spread areas, color).
Configuration of abdomen: normal (in lying or standing position), irregular – swollen.Considerable assymetry of the abdomen (in what area).Describe the presense of collaterals in the anterior wall of the abdomen.
Percussion of the abdomen:
Describe the degree of timpanite, if dull sound is determined, differenciate its source . Differenciate the type of fluid in the abdominal cavity. If swollen abdomen is present estimate the symptoms of fluktuation with the object of determining fluid in abdominal cavity.
Palpation of the abdomen:
Superficial palpation of the abdomen is determined, tension degree of abdominal wall, constipations (for how many days).Dependence of constipation: on food character; diarrhoea (due to what: food, nervousness, exertion), time when diarrhoea appeared and how many times a day, if it is painfull, false urges. Divergence of muscles, symptoms of Mendel, Shetkin-Blumberg. Deep, sliding, methodical, respiratory palpation of abdomen according to Obraztsov- Strajesco.Accordingly intestines are palpated: sigmoid, caecum.Detailed characteristics is given to palpated areas: diametre, painfullness, flexibility, consistency, surface (flat or bumpy). Determining the position of large curvature of the stomach and intestines (by percussion method- stomach timpanit delimitation by the method of deep palpation of large curvature, by method of auscultation).
