REALEASE OF LIABILITY & INDEMNIFICATION
I _________________________ hereby release the Mt. Joy Ski Club from any and all liability, cause of actions for any loss, damage, injury, illness or harm of any kind or nature which may occur to me or my child arising from participation in any of the Mt. Joy Ski & Snowboard school programs.
In the event that a medical emergency exists, I hereby authorize Mt. Joy Ski Club to take such reasonable care for my child, in addition to calling for medical care for such emergency services and related transportation. I agree to be responsible for all the expenses and costs associated with any medical care and related transportation for my child and/or myself.
Any medical or physical condition of a student must be explained in an attached letter.
