"Primary" indicates whether the selected institution is the primary institution from which you earned your undergraduate or 

bachelor's degree, or the most credits, or completed the majority of your prerequisites. You must select only one institution 

listed in the Institution List section as your primary school.  

 

Is this your current school?

 

Indicate whether or not you are currently attending the school, or if you intend to apply.  

 

Dates of Attendance

 

Enter the beginning and end (or anticipated end) dates of attendance for each school listing. Multiple degrees should all be 

under one entry. 

 

Degree Information

 

First Major:

 Enter your official major course of study at the institution, if applicable. Select "No Major" if none.  

 

Second Major:

 Enter second official major at the institution, if applicable.  

If your major is not listed, select “Other” and list it in the explanation statement. You will list minors in the explanation 

statement as well.
