The Diocesan Media Center serves the mission of religious education and lifelong faith formation by providing VHS and DVD rentals, as well as online links to resources in a variety of ministries and ages; our categories include: Archives, Biography, Catechetical, Children, Church, Ecumenism, Evangelization, Feature Films, Liturgical, , Multiculturalism, Peace/Justice, Prayer/Spirituality, Sacraments, Safe Environment, Scripture and Spanish. One of our great services is helping your parish ministries, catechetical leaders and schools, find appropriate and creative ways to use media for all ages. PAY AS USED: Open to all parishes in the Diocese of Joliet. Rental charges are applied to those wishing to use the media center on an occasional basis. Rent is $4.00 per video-one week rental plus shipping charges if applicable. All shipping is UPS and rates can be requested beforehand. Without return service, you may return your materials in at the front office before and after hours until 9:00pm. You can request to pick up your materials in the front office..

UNLIMITED USE MEMBERSHIP - $120.00

Unlimited one week free rental - all shipping is optional and extra.

ONE-WAY SHIPPING MEMBERSHIP - $270.00

Includes unlimited free one week rental and shipping to one parish address only.

TWO WAY SHIPPING/TOTAL USE MEMBERSHIP- $380.00

Includes unlimited free one week rental and all shipping (to and pick up from) one agreeable shipping address.

When ordering give the following information:

Series: If the catalog indicates that the video is part of a series, list the series title.

Title: List the specific title of the video(s) being ordered.

Bar Code: List the identification number which appears in parenthesis under the title i.e. (CVCX 0099)

INSTRUCTIONS FOR SHIPPING

Listing the video showing dates insures that videos arrive on time and return for other users.

Call for an extension of time if necessary.

Before using this form, be sure that the media center knows which membership plan you have selected.

Items can be returned at the front desk until 9pm during the week. Call if you are unsure. Please feel free to reproduce this sheet / two sided for larger orders.
