Purpose of the Job:

To educate and engage with local residents and community groups about health and how to improve their wellbeing
-
To support local residents to change their behaviour in order to improve their health and wellbeing
-
To provide focused support for local residents and community groups affected by health inequalities
Main Areas of Accountability:
-
To identify and engage with local individuals and community groups through a coordinated, service wide approach
-
To focus on working with individuals and community groups who experience comparatively poor health
-
To organise and participate in health promotion community events
-
To build up and maintain a network of contacts within the local community
-
To value the diversity and equal treatment of individuals
-
To engage service users in the development and evaluation of the Health Trainer service
-
To educate individuals and community groups about lifestyle choices and help individuals to identify how their behaviour might affect their health and wellbeing

To help individuals develop a personal health plan to identify the changes they want to make
-
To support individuals in achieving their personal health goals
-
To support individuals in maintaining their behaviour change
-
To signpost individuals to other agencies for information, support and resources to improve their health and wellbeing
-
To respond effectively to self-referrals and referrals from colleagues and partner organisations
-
To plan time and activities around the needs of clients
-
To keep up-to-date records of the work undertaken and entering records weekly on the national and organisation’s data base systems

To alert the line manager to issues arising

To take an active part in developing own knowledge and skills

To seek advice and support as and when appropriate

To monitor and maintain health, safety and security of self and others
-
To monitor and replenish stock levels of office supplies and work equipment

To follow both service and organisational policies and procedures

To inform the line manager about potential service improvements and opportunities to address any barriers to client’s life-style changes and health and wellbeing

To assist with any other requests from line management
Personal Specifications:

Fluent written and spoken English
-
Awareness of the limits of one’s own skills and ability to work within them

Able to talk confidently and effectively to people from a wide range of backgrounds

Able to listen face-to-face and on the phone

Good IT skills

Able to use the national DCRS Health Trainer database to record contact with local people and clients

Respectful and encouraging of people regardless of their different backgrounds

Able to respect confidentiality even in difficult situations

Able to identify, assess and manage risks

Able to manage own time and workload in order to reach individual and service targets

A flexible approach to work and an ability to work unsocial hours and/or in a variety of locations, as required

Qualifications and Knowledge:
-
City and Guilds Level 3 Health Trainer accreditation (or equivalent)
-
At least one year’s experience of volunteering or supporting your local community
-
Previous experience of working as a Health Trainer

Commitment to learn and undertake further training, such as Health Check training
-
Good numeracy and literacy skills
-
A strong interest in local communities in Kensington and Chelsea
-
Knowledge of researching and identifying local services and supporting individuals to use them
-
Knowledge of engaging and working with individuals and community groups
-
Knowledge of methods to facilitate behaviour change
-
Knowledge of one or more local community languages, at a colloquial level
-
Knowledge of lifestyle factors, health inequalities and barriers that contribute to health outcomes
