Applications under s 15 - commencement 
3.1. 
An application is to be made by affidavit, accompanied by a draft 
warrant in accordance with the Regulations. The formatting 
requirements for the affidavit are set out in Practice Direction 1.2.1

The affidavit must include information as to the authority of the 
applicant to make the application.

(a) 
to give notice of a pending application and an estimate of the 
time of delivery of the application;
(b) 
to request that arrangements be made for a Judge to hear the 
application;
(c) 
to inform the Coordinator (Listings) whether there are multiple 
applications and whether a particular Judge has previously had 
the conduct of the pending or related applications; and 
(d) 
to inform the Coordinator (Listings) whether the applicant 
proposes to exhibit videotape or other recordings to the 
affidavit so that (if necessary) arrangements can be made for the 
Judge to view or hear the recordings before the application is 
heard. 
3.4 
The Coordinator (Listings) will inform the applicant of the SDA 
number allocated to the application as soon as practicable, and usually 
during the first contact with the applicant.
3.5
The applicant must place the application in a sealed envelope, and 
must record on the envelope the SDA number that was allocated by 
the Coordinator (Listings) and the date of the application.
3.6 
There can be only one application in each envelope. If multiple 
applications are to be heard by one Judge on the same day, each 
application must be in a separate sealed envelope and each envelope 
must have the SDA number and date recorded on it.
3.7 
Once a Judge has been assigned to hear the application(s), the 
Coordinator (Listings) will as soon as practicable inform the applicant 
as to the identity of the Judge, the floor number of the Court on which 
the Judge's chambers are located, the time of the hearing and the 
contact details for the Associate to the Judge. 
3.8 
The applicant must go to the floor on which the Judge's chambers are 
located, contact the Associate to the Judge, and then hand the sealed 
envelope containing the application to either the Judge hearing the 
application or their Associate.

The Coordinator (Listings) will record the date, the Judge assigned 
and the SDA number in a register kept for that purpose.
