Scholarship Application

One time scholarship value: $500, Application Deadline: April 13, 2017

Eligibility: Must have been enrolled in CJHS band. Must be a graduating high school senior in a college performing instrumental music activity in the Fall of 2017. Recipient is not required to be a music major. Recipient must enroll in at least 12 college credit hours.

Application will be evaluated by a committee of the Carl Junction High School Boosters. The scholarship will be awarded based on this completed application, one recommendation form. The scholarship will be awarded at the awards assembly.

Please list your musical resume, then briefly list your school, community, and work activities, and work activities. Note any offices and/or honors received. (Attach a separate sheet if needed).
